Reports

This page outlines the reports that are available on the EMPOWER Online Portal.

How to Run a Report

  1. Log into the Employer Portal.
  2. On the left side of the screen, click the Reports icon.
  3. Click the +New report button.
  4. Click on the report that you wish to run.
    1. Select the settings for the report on the next page.
    2. Check the Email me when the report is available to receive an email when the report is ready.
    3.  Click the Request button. You will receive an email when it is ready.

Available Reports

Account Balance

This report shows participant level data for enrollment, contributions, and claims paid. Please note that this report does not include termed employees.

Description:

View plan account balance information per participant and per plan as of a specified date.

Settings:

  1.  File type: Choose pdf or excel.
  2. Time: Select the as-of date.
  3. Plan: Select the Plan year and plan that you’d like to use.
  4. Group by: Select Do not Group.
  5. Type: Select Detail to include social security numbers. Select De-Identified Detail to exclude social security numbers.

Fields:

  • Participant Name
  • Election
  • Employer Contribution
  • Rollover Contribution
  • Paid
  • Pending
  • Participant Deposits
  • Plan Year Balance
  • Available Balance
  • Cash Balance

Account Balance Detail

This report shows both a summary and detail level of enrollment, contributions, and claims paid. Unlike the Account Balance report, this report does show termed employees.

Description:

View plan balance summaries and consumer account balance detail as of specified date.

Settings:

  1. File type: Choose pdf or excel.
  2. Time: Select the as-of date.
  3. Plan: Select the Plan year and plan that you’d like to use.
  4. Group by: Select Do not Group.
  5. Type: Select Detail to include social security numbers. Select De-Identified Detail to exclude social security numbers.

Claim History

This report shows claims submitted during a selected time period. Use this report to check on the status of a claim.

Description:

View all claims submitted during a specified time period including status.

Settings:

  1. File type: Choose pdf or excel.
  2. Time: Select start and end date to filter by claim submission date.
  3. Plan: Select the Plan year and plan that you’d like to use.
  4. Group by: Select Do not Group.

Debit Card Funding

This report shows debit card activity over a selected date range.

Description:

View a summary of the debit card transaction amounts and fee amounts by plan.

Settings:

  1. File type: Choose pdf or excel.
  2. Time: Select start and end date to filter by transaction date.
  3. Type: Select either a summary (De-identified Summary), participant level without social security number (De-Identified Detail), or participant level with social security number (Detail).

Detail Tab

  • Identifier: unique identifier for participant
  • Participant Name
  • Card No: last four digits of the debit card
  • Transaction Date

Debit Card Mail Date

This report shows the date that debit cards were mailed to participants.

Description:

View the date(s) that cards were mailed to cardholders.

 

Settings:

  1. File type: Choose pdf or excel.
  2. Time: Select start and end date to filter by mail date.
  3. Card mail status: choose all to show cards that have been mailed or not yet mailed

  • Identifier
  • Participant Name
  • Cardholder Name
  • Card No: last four digits of the debit card
  • Mail Date
  • Debit Card Vendor

 

Debit Card Status

This report shows the status of participant debit cards.

Description:

View a list of the cards that have been issued for this employer.

 

Settings:

  1. File type: Choose pdf or excel.
  2. Card status: choose the status you would like to view. Choose All to see all statuses.

Fields:

  • Identifier: unique number for participant
  • Participant Name
  • Cardholder Name
  • Card No: the last four digits of the debit card
  • Status
  • Mail Date

Debit Card Transactions

This report shows Debit Card transactions.

Description:

View a list of all of the debit card transactions by settlement date.

 

Settings:

  1. File type: Choose pdf or excel.
  2. Time: Choose dates to filter the results by transaction date.
  3. Type: Choose to view the report with social security numbers (Detail) or without social security numbers (De-identified Detail).

Fields:

All Data is grouped by Settlement Date

  •  Identifier: unique number for participant
  • Participant Name
  • Card No: the last four digits of the debit card
  • Plan Year
  • Plan
  • Transaction Date
  • Status
  • Amount

 

Enrollment

This report shows participant enrollment as of a selected date.

Description:

View participant enrollment in applicable plans as of a specified date.

 

Settings:

  1. File type: Choose pdf or excel.
  2. Time: Choose the as-of date.
  3. Plan Year: Choose the plan year.
  4. Group by: Choose do not group.
  5. Type: Choose to view the report with social security numbers (Detail) or without social security numbers (De-Identified Detail).

HSA Account Detail

This report shows a summary of each participant’s HSA.

Description:

View an overview of each consumer’s HSA along with individual payroll deduction and employer contributions.

 

Settings:

  1. Time: Choose the date range.
  2. Group by: Choose do not group.
  3. Type: Choose to view the report with social security numbers (Detail) or without social security numbers (De-Identified Detail).

Fields:

  • Division: The Employer division the participant is assigned to
  • Identifier: Unique number for the participant
  • Last Name
  • First Name
  • MI: Middle initial
  • Employment Status
  • Employment Status Effective Date
  • Account Creation Date
  • Account Status
  • HDHP Coverage Level
  • IDV: The status of EMPOWER’s Identity Verification check. A status of N shows that the participant’s HSA is on hold and that they need to submit documents.

This report shows reimbursements and payments during a time period.

Description:

View all reimbursements/payments during a specified time period.

 

Settings:

  1. File Type; Choose PDF or Excel.
  2. Time: Choose the date range.
  3. Group by: Choose do not group.
  4. Type: Choose to view the report with social security numbers (Detail) or without social security numbers (De-Identified Detail).

Fields:

Summary Tab

  • Method: check, debit card, etc.
  • Amount

Detail Tab

Reimbursement Detail

This report shows reimbursed claims for a specified time period.

Description:

View all claims reimbursed during a specified time period.

  1. File Type; Choose PDF or Excel.
  2. Time: Choose the date range.
  3. Group by: Choose do not group.
  4. Type: Choose to view the report with social security numbers (Detail) or without social security numbers (De-Identified Detail).

Repayments

This report shows participant repayments.

Description:

View summary and detail of participant repayments for a specific time period.

 

Settings:

  1. File Type; Choose PDF or Excel.
  2. Time: Choose the date range.
  3. Group by: Choose do not group.
  4. Type: Choose to view the report with social security numbers (Detail) or without social security numbers (De-Identified Detail).

  • Repayment Method
